Congratulations to Associate Professor John M. Baratta, MD, MBA, on receiving the NCDHHS grant for the project Coordinating Resources to Enhance Stroke Transitions (CREST).

This creative project, which is a partnership with the UNC Department of Neurology, is to improve the coordination of clinical, social, and support services for people who have had a stroke or are at high risk, as well as to improve comprehensive treatment during the post-acute phase.
To create better and more consistent post-discharge stroke care, the initiative will make use of patient advisory groups, multidisciplinary stroke teams, and cutting-edge health information technology. In the US, stroke is still one of the primary causes of disability, and survivors frequently encounter major obstacles when trying to get access to social, medical, and rehabilitative services. About 700 people are discharged from the UNC Comprehensive Stroke Center each year.
